Services: Program for diagnosis and treatment of breast and cervical cancer
Eligibility: Applicant must be a Maryland resident that has a breast or cervical problem requiring further diagnosis & treatment; must meet income guidelines and insurance criteria. Women must be 40-64 years of age. Women over 65 eligible if no Medicare part B. Visit program's website for complete eligibility guidelines.
Application Requirements: Applicants must provide proof of residency and proof of income.
How to Apply: Applications can be obtained at the local health department or from the Maryland Department of Health and Mental Hygiene, Division of Cancer Control. Applications can also be completed at the offices of participating providers.
Benefits: Breast and cervical cancer diagnostic procedures (including ultrasound, biopsy, colposcopy, surgical consultations, etc), breast and cervical cancer treatment procedures (including cryotherapy, laser hysterectomy, lumpectomy, mastectomy, radiation therapy, and chemotherapy), physical therapy, occupational therapy, home health nurse, medications, medical equipment, breast prosthesis and bras, wig, breast reconstruction (nipple reconstruction/tattooing NOT covered), other costs related to diagnosis and treatment (lab tests, x-rays, hospital care)
