Eligibility Guidelines

How to apply

Applicants must have a child under age 19 (a close relative) who lives in the home; resident of Alabama, U.S. citizen or eligible immigrant, low income and, cooperate in obtaining medical support payments There are 3 ways to apply: 1) in person at a local county health department, healthcare center or certain hospitals; 2) by calling the toll-free number for a paper application, or 3) online at www.insurealabama.org.

Application Process

Applicants need to provide proof of: Alabama residency and U.S. citizenship (or qualified immigrant status), income and certain expenses, pregnancy (if applicable). Applicants must provide this information to Medicaid within 10 days. Also, sign over any medical insurance or medical support benefits to the state.
