NeedyMeds/RxOutreach 2009/2010 Grant Program

NeedyMeds and RxOutreach are proud to announce a subsidized grant program for health care clinics and other non-profit organizations that serve PAP participants. RxOutreach and NeedyMeds will underwrite the monthly cost of PAPRxTracker for qualifying organizations for 12 months.

Our goal is to increase the number of participants enrolled in patient assistance and deep discount programs. PAPRxTracker software automates application completion, allowing volunteers and staff to focus on patient enrollment, outreach, counseling, and advocacy rather than repetitive paperwork.

2009/2010 incentive grant:

Helps underwrite PAPRxTracker monthly charges for twelve months. Software charges are based on the standard charge of $2 per patient per month. Subsidies are as follows:

First 6 months: grant pays 100% of software cost
Next 3 months: grant pays 70%; grantee responsible for 30%
Final 3 months: grant pays 30%; grantee responsible for 70%
At end of grant period, grantees that subscribe to PAPRxTracker will be invoiced at the same rate as all other PAPRxTracker customers.
Grantees must agree to seek funding to replace grant support for electronic software.

NeedyMeds will provide coaching and technical support to grantees on an on-going basis to help:

Identify potential funders and prepare strong funding proposals
Use PAPRxTracker software

Eligibility criteria
Applicants must meet the following criteria:

Serve patients or clients for free or a modest fee to cover administrative cost.
Complete PAP forms by hand
Have identified an unmet need for PAP services
Will take action to increase the number of PAP patients/clients
Seek funds to continue use of PAP software after grant ends
Provide program updates as requested
Keep PAPRxTracker software and documentation confidential
Have access to a PC with Windows operating system, Adobe Acrobat and Internet Explorer

Application Process

The NeedyMeds Incentive Grant application has two required parts:

An application cover letter on organization letterhead, signed by the individual who will be directly responsible for administering the grant proposal, and that includes-:

A three- or four-paragraph statement that succinctly describes your program's:
- Mission
- Governance
- Target population and eligibility criteria
- Number of persons currently served
- Sources of financial support
A description of how PAPRxTracker will help you to increase the number of PAP participants you serve
A description of how you will identify funding to continue electronic PAP software

A completed NeedyMeds Incentive Grant application form

The application form must be completely filled out and signed by the individual responsible for administering the grant proposal. The cover letter and application must be submitted via email to incentivegrant@needymeds.org or by FAX (1-207-433-1036).

Applicants will be notified of funding decision no later than 30 days after the date of submission
