Eligibility Guidelines and Notes

The patient must be uninsured and meet income guidelines that are not disclosed.   This program provides a limited supply of ACCU-CHEK Aviva test strips to a network of community clinics and health centers throughout the U.S. to distribute to their low-income and uninsured patients with diabetes. There is no paperwork to fill out or registration process; however, clinics are subject to their own participation guidelines In order to receive the products, applicants must be a qualified registered patient at on the the identified clinics.
