    Details
    • Diagnosis of end stage renal disease
    • Transplant patient
    • Currently undergoing dialysis
    • Must be in financial need
    • Have documentation that the patient received Medicare/Medicaid or has applied for such
    • Program of last resort
    		Provides financial assistance for dental care for individuals who have end stage renal disease, kidney disease or to transplant patients.
